摘要:

目的 靜息代謝率(resting metabolic rate,RMR)預(yù)測(cè)公式是臨床營(yíng)養(yǎng)支持、防治肥胖中控制能量平衡的重要工具,而我國(guó)非肥胖健康成人的最佳預(yù)測(cè)公式尚不明確。本文將評(píng)估14個(gè)常見RMR預(yù)測(cè)公式的準(zhǔn)確性,尋找適合該類人群的最佳預(yù)測(cè)公式。方法 招募了46名非肥胖的健康成年人作為受試者,利用間接測(cè)熱法測(cè)量受試者的RMR,同時(shí)利用14個(gè)預(yù)測(cè)公式對(duì)其RMR進(jìn)行估算。通過(guò)偏倚、準(zhǔn)確率、均方根誤差和一致性相關(guān)系數(shù)評(píng)估每個(gè)預(yù)測(cè)公式的準(zhǔn)確性。結(jié)果 雖然H-B、Mifflin、WHO、Schofield、Livingston、Henry、Muller、Lazzer、De Lorenzo和Huang公式的偏倚較小(<5%),但個(gè)體層面的誤差較大,準(zhǔn)確率均不超過(guò)70%。預(yù)測(cè)準(zhǔn)確率依次是Henry(69.6%)>Lazzer(67.4%)=H-B(67.4%)>Mifflin(65.2%)=Schofield(65.2%)=WHO(65.2%)=De Lorenzo(65.2%)>Muller(63.0%)>Livingston(60.9%),Bernstein公式(21.7%)的準(zhǔn)確率最低。結(jié)論 對(duì)于我國(guó)非肥胖健康成年人,常見的14個(gè)RMR預(yù)測(cè)公式的準(zhǔn)確性均不理想,相對(duì)而言,Henry公式準(zhǔn)確性最高。在缺乏間接能量測(cè)試儀的情況下,Henry公式可作為替代方法。

Objective Predictive equations of resting metabolic rate(RMR)are important tools for control of energy balance in nutrition support and the prevention and treatment of obesity.However,the best one for non-obese healthy Chinese adults is unclear.Our objective is to examine the accuracy of 14 common RMR predictive equations and to find the best alternative for indirect calorimetry.Methods RMR was measured by indirect calorimetry and calculated by 14 predictive equations in 46 participants.Bias,accuracy rate,root mean squared prediction error and concordance correlation coefficient were used to evaluate accuracy of the predictive equations.Results Although the biases of H-B,Mifflin,WHO,Schofield,Livingston,Henry,Muller,Lazzer,De Lorenzo and Huang equations were small(<5%),they were less accurate in the individual level.All of the accuracy rates were no more than 70%.The Henry equation(69.6%)had the highest accuracy rate,followed by Lazzer(67.4%),H-B(67.4%),Mifflin(65.2%),Schofield(65.2%),WHO(65.2%),De Lorenzo(65.2%),Muller(63.0%)and Livingston(60.9%). Bernstein equation was the most inaccurate.Conclusions All of the 14 common predictive equations were not accurate enough in non-obese healthy Chinese adults.Henry equation was relatively the most accurate and may act as the best alternative for indirect calorimetry when indirect calorimetry was unavailable.
