摘要:

目的 膈肌表面肌電信號是評價呼吸功能的客觀指標之一, 在采集過程中, 電極放置位置的不同對膈肌表面肌電信號的質(zhì)量有很大的影響, 因此需要對放置采集電極的位置科學定位以獲得較好質(zhì)量的膈肌肌電信號。方法 首先以某健康受試者的深呼、深吸兩相下的CT圖像為依據(jù), 通過醫(yī)學建模軟件Mimics重構膈肌深呼、深吸兩相下的模型, 測量并對比各自的膈肌長度參數(shù), 獲得深呼、深吸兩相下膈肌與肋骨的公共重合區(qū)域即對合區(qū)域, 以此確定體表采集位置的范圍。其次, 設計膈肌表面肌電信號采樣點實驗, 在上述位置上, 采用64通道陣列電極獲得同一健康受試者深呼吸時的肌電信號, 計算各采樣點去除心電噪聲后的運動/靜息比 (Rij) , 并以此評價所采集的膈肌表面肌電信號的質(zhì)量。結果通過采樣點實驗驗證膈肌模型與Rij分布的一致性, 進一步確定了膈肌表面肌電信號采集的較佳位置。結論 人體右側(cè)腋前線與第8~9根肋骨間, 采集的膈肌肌電信號質(zhì)量較好, 并且略遠離心臟的右側(cè)區(qū)域采集到的信號更好。

Objective The measurement of electromyography activity of diaphragm is one of the objective indicators to evaluate respiratory function, yet the electrode position during the acquisition is a great impact on the quality of diaphragm electromyography ( EMGdi) . Therefore, it is necessary to scientifically locate the position where the electrodes are placed. Methods Firstly, with the CT image of a healthy subject under deep breath, the diaphragm model of the deep-breathing position is reconstructed by the medical modeling software Mimics, the confinement area of the diaphragm is obtained by comparing the length parameter of the two-phase and the range of the electrode location is determined. Secondly, sampling point experiment of the EMGdi signal is designed, EMGdi signals of the same healthy subject are obtained in the aquisition positions with 8×8 array electrodes, the restoring period is used as the reference to calculate the motion/rest ratio after removing the ECG noise, and the quality of EMGdi signal based on the motion/rest ratio is evaluated. Results The consistency of the biomedical model and the distribution of Rijon array electrodes is verified by the sampling point experiment, and the precise position of the EMGdi signal acquisition is further determined on the basis of modeling. Conclusions The area between the intersections of 8-9 ribs of right side of the body surface is a good position for collecting the EMGdi signals and a little area on the right side is better.
