摘要:

心電圖是心肌缺血檢測的一種方便、經(jīng)濟(jì)、無創(chuàng)的工具,其主要的臨床表現(xiàn)為ST-T段變化。鑒于心
臟是一個混沌系統(tǒng),本文從非線性角度出發(fā),對心電圖特征參數(shù)序列進(jìn)行分形維數(shù)和最大Lyapunov指數(shù)
分析,其中特征參數(shù)序列包括:心電圖RR間期,瞬時心率HR(heart rate),ST段的平均幅值STmean,ST
段最小幅值STmin,STmean與HR的比值STr,T波峰末間期TpTe,R波峰值Ramp,T波峰值Tamp。利用公共數(shù)
據(jù)庫Long-term ST database(LTST)中心肌缺血和非心肌缺血心電數(shù)據(jù),通過統(tǒng)計t檢驗(yàn)分析,除最大分
形維數(shù)分析中的TpTe與最大Lyapunov指數(shù)分析中的Ramp和Tamp特征參數(shù)外,其余參數(shù)均有顯著性差異。
基于心電圖的非線性分析為心肌缺血檢測與診斷提供另一重要依據(jù)。

Electrocardiogram (ECG) is a convenient, economic, and non-invasive detecting
tool for myocardial ischemia (MI), and its clinical appearance is mainly exhibited by ST-T
complex changes. In view of the fact that the heart is a chaos system, the nonlinear
parameters of fractal dimensions and largest Lyapunov exponents were used to analyze some
derived feature sequences in ECG in this paper. These feature sequences included ECG RR
interval, transient heart rate (HR), average amplitude of ST-segment (STmean), minimum
amplitude of ST-segment (STmin), the rate of STmean and HR (STr), T apex T end (TpTe), R-
wave peak amplitude (Ramp), and T-wave peak amplitude (Tamp). We statistically analyzed the
ECG signals of patients with myocardial ischemia and healthy persons from long-term ST data
base (LTST) with T test, and as a result, significant differences of all feature sequences
were obtained except the feature of TpTe by fractal dimensions analysis, Ramp and Tamp by
Lyapunov exponents analysis. Therefore the nonlinear analysis based on ECG provided a
reliable basis for detection and diagnosis of myocardial ischemia.
