摘要:

目的  利用3D磁共振圖像研究輕度認(rèn)知障礙(mild cognitive impairment,MCI)患者首次掃描和3年后隨訪(fǎng)數(shù)據(jù)的全腦灰質(zhì)體積改變。方法  利用基于體素的形態(tài)測(cè)量學(xué)(voxel-based morphometry,VBM)方法對(duì)19例ADNI數(shù)據(jù)庫(kù)中符合條件的男性MCI患者T1結(jié)構(gòu)像進(jìn)行預(yù)處理,得到平滑后全腦灰質(zhì)圖像,并對(duì)首次掃描數(shù)據(jù)和3年后隨訪(fǎng)數(shù)據(jù)的全腦灰質(zhì)圖像進(jìn)行基于體素的統(tǒng)計(jì)學(xué)比較。此外,通過(guò)分析3年后隨訪(fǎng)數(shù)據(jù)中的臨床統(tǒng)計(jì)學(xué)特征,將實(shí)驗(yàn)樣本分為穩(wěn)定型MCI(stable MCI,SMCI)和進(jìn)展型MCI(progressive MCI,PMCI),并將SMCI和PMCI首次掃描數(shù)據(jù)進(jìn)行全腦灰質(zhì)比較。結(jié)果  與首次掃描數(shù)據(jù)相比,3年后隨訪(fǎng)數(shù)據(jù)中雙側(cè)小腦后葉、右側(cè)小腦前葉、右側(cè)顳葉、右側(cè)顳中回、右側(cè)額下回和右側(cè)海馬旁回等區(qū)域的灰質(zhì)體積發(fā)生萎縮。與SMCI組相比,PMCI組中左側(cè)小腦后葉、右側(cè)枕葉、右側(cè)前扣帶區(qū)域顯示灰質(zhì)體積均有顯著性差異(P<0.005)。結(jié)論  利用VBM方法對(duì)MCI患者磁共振圖像分析能夠客觀揭示MCI患者特定區(qū)域灰質(zhì)體積萎縮,為阿爾茨海默病的早期診斷提供可靠的影像學(xué)依據(jù)。

Objective To explore the difference of grey matter&rsquo;s volumes between baseline and 3-year follow-up of mild cognitive impairment(MCI)patients in 3D magnetic resonance image.Methods Based on the method of voxel-based morphometry(VBM),we processed the 3T subjects(n=19)of MCI at baseline and 3 years later obtained from ADNI.After image preprocessing,we got the smoothed whole brain gray matter(GM)and then tested the changes of GM in the three years.In addition,based on the analysis on the clinical statistics characteristics of 3-year follow-up data,the samples were divided into two groups as progressive MCI(PMCI)and stable MCI(SMCI).Then we compared the whole brain GM difference between PMCI and SMCI.Results The regions of GM loss over the 3-year follow-up were the bilateral cerebellum posterior lobe,the right cerebellar anterior lobe,the right temporal lobe,the right middle temporal gyrus,the right inferior frontal gyrus and the right parahippocampal.Compared with SMCI,PMCI group had GM loss in the left cerebellum posterior lobe,the right occipital lobe and the right anterior cingulate regions(P<0.005).Conclusions Based on magnetic resonance images,the VBM method could objeetively reveal the GM atrophy in certanin regions and these results provided some reliable imaging basis for the early diagnosis of Alzheimer&rsquo;s disease.
