摘要:

微波或射頻熱消融已成為腫瘤治療的一種重要手段。實時準確地監(jiān)測腫瘤熱消融溫度和熱凝固區(qū)變化是保證治療安全、取得良好療效的關鍵。腫瘤熱療超聲無創(chuàng)監(jiān)測技術一直是國內(nèi)外研究的熱點。超聲彈性成像作為一種新的超聲組織定征技術,在腫瘤熱療無創(chuàng)監(jiān)測方面正受到越來越多的關注。本文介紹了基于靜態(tài)彈性成像、聲輻射力彈性成像以及運動電極彈性成像進行腫瘤熱療無創(chuàng)監(jiān)測的原理及方法,概述了國內(nèi)外研究現(xiàn)狀,并從以下方面總結(jié)了發(fā)展方向:如何實現(xiàn)活體內(nèi)監(jiān)測、如何結(jié)合現(xiàn)有的商業(yè)超聲診斷系統(tǒng)、如何實現(xiàn)測溫和凝固區(qū)監(jiān)測的有效結(jié)合、如何提高抗運動偽影能力,以及如何提高監(jiān)測的實時性和精確性。

Microwave and radio-frequency thermal ablation has become an important means for tumor treatment. Real-time and precise monitoring of the change of temperature and thermal coagulation zones in thermal ablation of tumors is the key to ensure the safety of treatment and to achieve satisfactory curative effect. The ultrasound-based noninvasive monitoring techniques for thermal therapy are always a hot point of research. As a new ultrasonic tissue characterization method, ultrasound elasticity imaging is paid increasing attention in respect of the noninvasive monitoring of thermal therapy. In this paper, we introduce the principles and methods of noninvasive monitoring based on static elasticity imaging, acoustic radiation force elasticity imaging, and electrode motion elasticity imaging. We also summarize the state of the research home and abroad, and the prospect in these respects: how to achieve in vivo monitoring, how to incorporate the use of existing commercial ultrasonic diagnosis systems, how to combine temperature measurement with coagulation zone monitoring effectively, how to improve the capability of resistance to motion artifact ability, and how to increase the real-time performance and accuracy of monitoring.
